The FORM must be filled out and submitted in order to hold a seat at the seminar. Payment must be received at least 30 days in advance to confirm your reservation. You will receive confirmation of a space in the class within one week after submitting the form, and again after payment is received. If you have not received confirmation please call or e-mail us.
